Most of it is preloaded from the plan

If the actual access times differ from the plan, explain why and categorise the reason. If work was unplanned, log it under the correct contract category. Non-productive time gets a reason before you save. Those categorised reasons are what a QS filters on later.

Submit checks that all six steps are complete, locks the record and sends it for review. One shift, one record, signed off before you leave site. That is what a contemporaneous record means in practice.
